Electronic Reporting

For reporting leaks, spills, fires, etc.
Operators can submit electronic reports
of undesirable events (oil and natural gas leaks, spills, fires,
etc.) to the division. To know more, click here.
Operators may also amend existing reports previously entered into
the system. Original reports should be amended when corrections
are needed, rather than creating new reports for the same event. |

This program allows oil and gas operators to electronically submit Applications for Permit to
Drill (APD), Sundry Notices, Well
Completion Reports, and Workover Tax Credit
applications to the Utah Division of Oil, Gas and Mining.
PDF copies of well logs are now being accepted online.
Operators may submit such logs via our ePermit program through
the PDF Well Log option (on the main menu) or as
attachments to a Well Completion Report. Operators submitting
their well logs as PDF images will no longer be required to
submit paper copies of their logs to the Utah Division of Oil,
Gas and Mining.
An ePermit registration form (below) must
be submitted prior to using this system.
NOTE: If you encounter problems with ePermit or
have questions, please contact us via email at
OG-ePermit@utah.gov. Please include the number of the APD or Sundry you are working on, if available.

The Utah eForm program allows oil and gas operators to
submit the following monthly reports electronically to the
division: - Oil and Gas Production
Reports (Form 10), - Disposition Reports (Form 11),
- Gas Plant Reports (Forms 13A & 13B), - Injection
Reports (UIC Forms 2 & 3)

Register to use eForm to electronically submit production, disposition, gas plant, and injection reports.
Anyone who was registered to use the old eReport program should automatically be registered to use eForm. A new user must register by completing 2 steps:
1. Complete the "Need to add a User? - Register User" option on the eForm Login page;
AND
2. (UNDER CONSTRUCTION) Complete this registration form and mail it to the Utah Division of Oil, Gas and Mining. The original, notarized form must be submitted. Copies will not be accepted.
Once division personnel receive both parts of the registration and approve it, the new user will be allowed to access the system.
