Incident Report Form	For reporting leaks, spills, fires, etc. Operators can submit electronic reports of undesirable events (oil and natural gas leaks, spills, fires, etc.) to the division. To know more, click here.
Utah ePermit - Login	SUNDRY NOTICES can now be submitted electronically!! Login to file drilling permits and/or submit sundry notices electronically. This program allows oil and gas operators to electronically submit Applications for Permit to Drill (APD) to the Utah Division of Oil, Gas and Mining. A registration form (below) must be submitted prior to using this system.
Utah ePermit Registration Form	Register to electronically apply for permits and submit reports. A company must register to use the ePermit system by completing this form and mailing it to the Utah Division of Oil, Gas and Mining prior to electronically submitting Applications for Permit to Drill (APD), Sundry Notices , or Well Completion Reports (coming soon). Form was updated 4/22/2009.
Utah eReport - Login	Login to report oil and gas production. The Utah eReport programs allows oil and gas well operators to submit Monthly Oil and Gas Production Reports, Disposition Reports, and Injection Reports electronically to the division.
