Incident Report Form	For reporting leaks, spills, fires, etc. Operators can submit electronic reports of undesirable events (oil and natural gas leaks, spills, fires, etc.) to the division. To know more, click here.
Utah ePermit - Login	Login to file for drilling permits electronically. This program allows oil and gas operators to electronically submit Applications for Permit to Drill (APD) to the Utah Division of Oil, Gas and Mining. A registration form (below) must be submitted prior to using this system.
Utah ePermit Registration Form	Register to file drilling permits electronically. Prior to submitting electronic Applications for Permit to Drill (APD), a company must register to use the ePermit system by completing this form and mailing it to the Utah Division of Oil, Gas and Mining.
Utah eReport - Login	Login to report oil and gas production. The Utah eReport programs allows oil and gas well operators to submit Monthly Oil and Gas Production Reports, Disposition Reports, and Injection Reports electronically to the division. Clicking here will take you to the eReport login screen. Click on "Utah eReport Assistance" (below) for more information about the program. NOTE: The option for submitting large BATCH REPORTS is temporarily OUT OF ORDER. This method of converting company data to XML code has proven to be too complex for most operators and we have abandoned it. We hope to develop a simpler application sometime in the future, hopefully during 2008. We apologize for this situation.
